How to build healthy habits for sustained success?

I’m posting from a throw-away account to preserve my anonymity. I am 24 years old and am in the worst shape of my life. I used to be a competitive soccer player and ever since a series of nasty injuries several years ago, have become increasingly sedentary. I am now 285 lbs and consider myself to be nearly 70 lbs away from my target weight (I am 6’4” and have a stocky build). I know that the key to sustaining health goals is by building healthy habits but I can’t seem to drill these into my head.

I have done IF and have pursued gym routines which have yielded results, but I seem to derail entirely within a few months.

Does anybody have practice tips/experience in how to build and keep these habits? What can I do to mitigate the risk of future relapses? Any words of advice would be greatly appreciated.
